We determine how the carpet got wet and how long it has been wet. Clean-water exposure caught quickly has different options from sewage contamination or long-standing dampness.
Portable extraction equipment removes retained water before any removal decision, reducing the volume carried through the property.
Carpet pad is removed in all affected areas. Tack strip is replaced where it has been saturated, since it holds moisture directly against the subfloor edge.
The exposed subfloor is cleaned, treated with an appropriate antimicrobial and dried with directed airflow and dehumidification.
Salvageable carpet is handled per its condition; contaminated or long-saturated carpet is removed. The decision is made on the readings and the water category, not on appearance.

Sometimes. Clean-water exposure extracted and dried within roughly the first day or two often allows the carpet to stay, with the pad replaced. Carpet contaminated by sewage, or wet for an extended period, is removed.
Pad is highly absorbent and dries far slower than the carpet above it. Left in place, it keeps the subfloor wet and continues to support growth no matter how well the carpet is cleaned.
Not when the odour is coming from the pad or subfloor. Cleaning the face fibre addresses the layer that is not the problem, which is why the smell comes back as soon as the carpet dries.
Below-grade carpet over a damp slab tends to recur even after drying, because the slab keeps delivering moisture. Moisture control at the slab has to be part of the solution.
